Critical Service Request

IMPORTANT – This form is ONLY to be used for CRITICAL IT support requests. Any other types of requests (including patient requests for service) will not be answered.

A critical service request is only to be used if you have a problem that needs to be solved immediately. If there is a workaround for right now, please fill out a regular request here.

IS THIS ACTUALLY A CRITICAL SERVICE REQUEST?

When you send in the critical service request, everyone in OCIS will stop what they are doing and respond to the critical service request.

Please only submit critical service requests if the problem needs to be solved immediately and there is no workaround or other computer or printer to use.
