By Mail: Complete and print the application form with the names of all participants for each course and enclose a check for the full fee in U.S. currency (only) for each course. Make checks payable to UNC School of Dentistry.

PAYMENT AND CONFIRMATION

Full payment of the registration fee is due at the time of enrollment for all courses. Upon enrollment, you will receive a confirmation letter that will include directions, parking information, registration time, and receipt for tuition paid. You are encouraged to register early to ensure receipt of your confirmation letter prior to the course.

CANCELLATION AND REFUND POLICY

Refunds, less a $30 administrative fee, per person, will be granted if cancellation notice is received at least five working days prior to the start date of a program. Refunds cannot be given for “no shows” or cancellations received once the course has started. In the event the UNC School of Dentistry feels the necessity to cancel a program, the registrants will receive a full refund.

LIMITED ATTENDANCE COURSES

Some courses have limited attendance; participants are enrolled on a first-come basis. Special registration and cancellation policies may apply. Tuition discounts cannot be applied to limited attendance courses.

All courses are held in Chapel Hill, North Carolina unless otherwise indicated.
